Disability Documentation Requirements

Chimes is a participating non-profit agency under the AbilityOne® Program, which is among the nation’s largest sources of employment for people who have significant disabilities. The Program creates private sector jobs in every state for Americans with significant disabilities.

When preparing your proof of  disability documentation we require documentation verifying your disability. This documentation must be provided by your healthcare provider or a qualified government agency and should include the following:

  1. Medical diagnosis (not just a list of symptoms or a history report)
  2. Indication of whether your disability is permanent or non-permanent
  3. Any functional limitations
  4. Recommended support and accommodations
  5. Signature and date from your practitioner, including credentials, on official letterhead with the provider’s address and telephone number
  6. Confirmation that the documentation reflects a current disability, not a condition that has been resolved
  7. Please note: Schedule-A Certification letters are not acceptable unless the disability diagnosis is explicitly stated in the letter
  8. A benefit’s award letter from the Social Security Administration is acceptable.
  9. A letter from the Veterans Administration is acceptable. The letter must contain the breakout of your disability
